隱私權政策

「台灣短吻犬呼吸功能分級評估系統」（以下簡稱本系統）由國立臺灣大學獸醫專業學院小動物耳鼻喉暨臨床睡眠研究室（以下簡稱本研究室）管理營運。 我們重視您的個人資料保護，並依《個人資料保護法》蒐集、處理及利用您的個人資料。請您使用本系統前詳閱本政策。

一、蒐集之目的

• 辦理 RFG 呼吸功能分級評估之線上申請、評估、報告核發與查詢
• 帳號登入、身分驗證與權限管理（獸醫師、飼主、管理人員）
• 與您聯繫評估相關事宜
• 以去識別化方式進行學術研究與統計分析
• 系統安全維護與操作紀錄

二、蒐集之個人資料類別

• Google 帳號資料：以 Google 登入時，取得您的 Email、顯示名稱、大頭貼網址及帳號識別碼。本系統不會取得您的 Google 密碼。
• 飼主資料：姓名、手機號碼、Email、通訊地址。
• 犬隻資料：犬名、品種、性別、出生日期、晶片號碼、評估問卷作答及評估結果。
• 獸醫師資料：姓名、執照字號、聯絡電話、執業院所、資格證明文件及電子簽名。
• 系統紀錄：操作紀錄、IP 位址、瀏覽器資訊及存取時間。

Google 使用者資料之存取、使用、儲存與分享

本系統僅以 Google 帳號作為登入方式（Firebase Authentication），僅取得 Google 帳號之基本資料：

• 存取：Email、顯示名稱、大頭貼網址及 Google 帳號識別碼。不存取您的通訊錄、雲端硬碟、Gmail 或其他任何 Google 服務資料。
• 使用：僅用於辨識您的身分、建立及登入本系統帳號、於帳號選單顯示您的名稱與 Email，以及寄送評估相關通知至該 Email。
• 儲存：上述資料儲存於本系統之伺服器資料庫，並依使用者身分控管存取權限。
• 分享：不出售、不出租；除寄送通知信時須將收件 Email 交由電子郵件寄送服務（Resend）傳遞外，不提供予其他第三人；不用於廣告、行銷或訓練人工智慧模型。
• 刪除：您可隨時來信要求刪除帳號及相關 Google 使用者資料；您亦可至 Google 帳戶「第三方連結」頁面移除本系統之存取權。

三、利用期間、地區、對象及方式

• 期間：自蒐集日起至蒐集目的消失為止；依法令或研究需要須保存者，依其規定期間保存。
• 服務提供者：登入驗證由 Google（Firebase Authentication）提供；電子郵件通知由 Resend 寄送。上述服務可能於中華民國境外處理資料。
• 對象：本研究室、執行評估之合格獸醫師及其院所；分級結果以去識別化方式提供英國畜犬協會（The Kennel Club）資料庫。
• 方式：以電子方式處理及利用。除前述對象、服務提供者、法令規定或經您同意外，不會將可識別您個人之資料提供予第三人，亦不作商業行銷用途。

四、資料安全

飼主手機號碼、詳細地址及犬隻晶片號碼以加密方式儲存；網站使用 HTTPS 加密傳輸，並依身分限制資料存取範圍，系統操作留存紀錄以供稽核。

五、Cookie 之使用

本系統僅使用維持登入狀態所必要之 Cookie，不使用廣告或第三方追蹤 Cookie。

六、您的權利

依《個人資料保護法》第 3 條，您可就您的個人資料行使下列權利：

• 查詢或請求閱覽
• 請求製給複製本
• 請求補充或更正
• 請求停止蒐集、處理或利用
• 請求刪除

如需行使上述權利，請以下方聯絡方式與我們聯繫。若您不提供必要之個人資料，本系統將無法為您辦理評估及報告核發。

七、政策修訂

本政策如有修訂，將公告於本頁並更新「最後更新日期」。

English Summary

RFG Taiwan (https://rfg-taiwan.com) is operated by the Small Animal ENT & Clinical Sleep Laboratory, School of Veterinary Medicine, National Taiwan University. It manages Respiratory Function Grading (RFG) assessments for brachycephalic dogs.

Google user data. We use Google Sign-In (Firebase Authentication) and obtain only basic Google account information.

• Access: your email address, display name, profile photo URL and Google account ID. We do not access any other Google data (Contacts, Drive, Gmail, etc.).
• Use: solely to authenticate you, create and sign in to your account, display your name and email in the account menu, and send assessment-related notifications to your email.
• Storage: stored in the system's server database, with access restricted by user role.
• Sharing: we do not sell or rent Google user data. Apart from passing your email address to our email delivery provider (Resend) to send notifications, we do not share it with any other third party, and do not use it for advertising, marketing or training AI models.
• Deletion: email us to delete your account and associated Google user data. You may also revoke access at any time from your Google Account's “Third-party connections” page.

Other data. Dog owners' name, phone, email and address; dog name, breed, sex, birth date, microchip number and assessment results; veterinarians' name, licence number, phone, clinic, credentials and e-signature; and activity logs (IP address, browser, time). These are used only to provide assessments and reports, contact you, maintain security, and for de-identified research statistics. Owner phone numbers, street addresses and microchip numbers are encrypted at rest, and the website uses HTTPS. Login is provided by Google (Firebase Authentication) and emails are sent via Resend; these providers may process data outside Taiwan. We use only the cookie strictly necessary to keep you signed in; no advertising or tracking cookies.
